Recovery from alcohol and drug addiction is a life-long process.

Studies have shown that the first twelve months from the time a person receives care addressing their addictions is the most vulnerable period and that relapse is a likely factor.

Segue by Benchmark is a program born out of experience. Because Benchmark offers our residents the most effective program possible,  we provide a comprehensive aftercare program. Segue is a unique service, staffed by in-house Certified Recovery Coaches (CRC), providing twelve months of support and accountability that ensures a smooth transition from rehab to permanent sobriety.

Clients will work one-on-one with their personal CRC through a four stage transition. Additionally, their CRC will work directly with the client’s collaterals- family, therapists, legal interests, etc. This comprehensive approach has been proven to provide the type of insurance necessary to detect any relapse behaviors and address them in real-time, allowing any intervention and correction necessary.

 

 

 

 

 

Segue I: “Introduction” – During Discharge Planning from BRC

  • Review continuing care plan prior to discharge from BRC
  • Assist in identifying local support resources
  • Secure appointments with support resources
  • Review Segue program guidelines and expectations
  • Assist in contacting collaterals

Segue II: “Transition” – Months 1 through 3

  • Provide introduction to sober living
  • Prioritize immediate needs (issues / events / relationships)
  • Identify specific Recovery Network
  • Assist with Family/Support System Integration Issues

Segue III – “Stabilization” – Months 4 through 6

  • Formal Review of client progress to date
  • Ongoing relapse prevention monitoring through accountability
  • Implement personal development plan addressing
    • Emotional / Behavioral
    • Social / Recreational
    • Financial / Employment / Educational
    • Relationship / Spirituality

Segue IV – “Acclimation” – Months 7 through 12

  • Review personal development plan, modify if necessary
  • Prioritize and set attainable & measurable goals
  • Address any unresolved medical, legal or other issues
  • Provide budgeting assistance and/or credit counseling referral
